Gap between clinical trials and clinical practice: lessons from the Bypass Angioplasty Revascularization Investigation (BARI).

چکیده
McGuire and colleagues 1 present interesting data and a careful analysis of how physicians respond to clinical trial results. Utilizing data from the National Cardiovascular Network and surveys of investigators at 13 sites, the authors considered the impact of the original Bypass Angioplasty Revascularization Investigation (BARI) randomized trial results,2 and the National Heart, Lung, and Blood Institute Clinical Alert3 on the use of percutaneous coronary interventions (PCI) and coronary artery bypass surgery (CABG) for patients with treated diabetes. Their conclusion is that the BARI results had no significant impact on revascularization treatment selection in the BARI-eligible patients with diabetes. The implications of this negative result are of real concern, but some caution in generalizing the conclusions is appropriate.

Optimal Treatment for Myocardial Revascularization: Surgery or Stenting?
Since the introduction of coronary artery bypass grafting (CABG) in 1967, and percutaneous transluminal coronary angioplasty (PTCA) 10 years later, several major clinical trials have been conducted comparing the two therapeutic strategies, such as the Bypass Angioplasty Revascularization Investigation (BARI) (1) and the Coronary Angioplasty versus Bypass Revascularization Investigation (CABRI) ...

Clinical trials are organized for many different reasons. Most drug and device trials are designed to demonstrate the safety and efficacy of a drug or device, not to prove that they are the optimal treatments for patients. On the other hand, treatment strategy trials are designed to study the value of one therapeutic regimen compared with another when equipoise or confusion exists regarding whi...
متن کاملInfluence of the Bypass Angioplasty Revascularization Investigation National Heart, Lung, and Blood Institute Diabetic Clinical Alert on Practice Patterns Results from the National Cardiovascular Network Database
Background—In 1995, the Bypass Angioplasty Revascularization Investigation (BARI) found that patients with diabetes had a survival benefit when treated with surgical revascularization versus balloon angioplasty, prompting a National Heart Lung and Blood Institute (NHLBI) “Clinical Alert.” The influence of the BARI findings and of the Clinical Alert on practice patterns is unknown. Methods and R...
متن کاملNew section in Circulation: implications of contemporary clinical trials.
We are pleased to announce to our readers the launch of a new section in Circulation. The purpose is to provide a perspective on contemporary clinical trials in cardiovascular medicine. These trials need not be published in Circulation but must fulfill a minimum set of criteria so as to allow a multidisciplinary discussion of the trial across at least three of the following domains: clinical pr...
متن کاملImportance of considering atherosclerosis progression when choosing a coronary revascularization strategy: the diabetes-percutaneous transluminal coronary angioplasty dilemma.
The choice of which coronary revascularization strategy is best for diabetic patients with multivessel coronary disease has always been complex. The presence of diabetes, especially insulin-requiring diabetes, has been associated with higher rates of acute and late-term adverse events for both PTCA1 and bypass surgery,2 making either approach seemingly suboptimal. The results of the Bypass Angi...
